Displaying the chapter and page number together in Microsoft Word only seems difficult because the solution isn't particularly intuitive. If you know where to look, the solution is easy.

You’ll probably want the index page to have its own page numbering, similar to the chapter and appendix section. The easy answer is to make a section for the index by repeating the process in #9.
